GFHS’ core services include primary medical, behavioral health and dental care to children and adults at several locations across Catawba, Davidson, Gaston, Iredell and Lincoln counties.
Over the past decade, professionals in both the mental health and physical health communities have developed a strong interest in the integration of behavioral health services into the primary care medical system.
Recognizing the multiple needs in the communities we serve, GFHS has moved towards a fully integrated care model in order to provide supportive services beyond the scope of traditional primary care. Our integrated care model allows both primary care and behavioral health to work in a shared system.
GFHS’ Behavioral Health services are now provided at all medical practice locations.
GFHS’ Behavioral Health provider team, which includes Licensed Psychologists, an MD and Licensed Clinical Social Workers (LCSWs) are available during regular clinic hours, working with your medical provider, to address a variety of concerns including:
- Alcohol Use
- Chronic medical conditions*
- Energy Level
- Substance Use
- Tobacco Use
- Well Child Check Assessments
BH services are available at our Behavioral Health Outreach sites, where our behavioral health team providers are located within the partner organization’s clinic setting. Services include behavioral health support, substance abuse assistance and engagement in GFHS’ MAT program. For more information about GFHS’ Behavioral Health Outreach Sites, click here.
